Retrieval Preparation and Shipping 

 

Mark the retrieval according to specified orientation during revision surgery. Metal parts can be scribed (on non-important) surfaces or indexed with a sharp pointed instrument. Ceramics are the most difficult being impossible to mark the hard surfaces. Ink markers can function as temporary pointers until reviewed by the DARF lab.

 

Wrap each retrieval piece separately to protect the surfaces. A surgical glove can be used in addition to gauze or sponges. Our objective is to receive each retrieval in the same condition as when you shipped it to us.

 

You may use a variety of containers for shipping. Be sure to mark the containers with the identification information to avoid confusion and misidentification on arrival. 

 

Please complete the Implant Form (be sure to specify your implant orientation marks). The implant form can be included in your packaging. For international shipping please also complete the International Shipping Form and keep it with any other shipping documents for customs purposes.  Forms provided upon request.

Retrieval Reporting 

 

Though every effort to ship your retrievals safely and properly are made, DARF is not responsible for items lost or damaged during research or shipping.

 

Reporting of retrieval analysis follows 5 levels of detailed reporting: 

 

Level 1 consists of the DARF center sending you an acknowledgement receipt (PowerPoint file). Please review all initial reports for their accuracy. FREE 

 

Level 2 includes wear maps and possible stripe wear. $500 

 

Level 3 includes surface roughness (Zygo) and CMM results. $500 

 

Level 4 includes detailed SEM and EDS performed on marked areas of interest. $500 

 

Level 5 includes written consolidation of data as a case report. If you would like data consolidated for multiple retrievals please specify at receipt of level 1 report. $750
